Please agree to the below terms (if you are unable to agree to anything, please discuss with us before completing your application)*

We want our dog to have puppies

We own our own home

We have a fully enclosed garden

We live within a 1.5 hour drive (females) or a 20 minute drive (males) of Forever Doodles

We are willing to feed the diet approved by Forever Doodles (www.pawdogfood.com)

We are willing to adhere to care guidelines from Forever Doodles

We are willing to attend puppy training classes

We will openly and honestly communicate about all dog-related matters

We will allow Forever Doodles breeders to visit our home if or when required

We will cooperate with Forever Doodles to coordinate breeding the dog for up to 4 litters (females) / up to 6 years (males)

We will work with Forever Doodles to complete all required health screening

We are able and willing to transport the dog to Forever Doodles when required

We understand that Forever Doodles may require the dog to stay with them from time to time

We understand that we cannot opt the dog out of the guardian programme after joining
